I bought a funny "muscle" tee (designed to look like a hole was cut in the shirt and the body underneath was ripped) as a gift, but when I recieved it a week later the picture was really small and damaged, and the tee itself had a teeny neck-hole. I emailed zazzle and included a photo- they gave me a full refund and didn't even want the shirt returned. A waste of time, and I had to rush to find a new gift, but at least I didn't lose any money over it.
Zazzle is great for customising anything you can really name, and this being the first time I'd used them I was a bit wary about the quality.
Having said that, they make it really easy to get exactly what you want, the design tools are easy to use, and they provide you with documentation and emails at every step, along with a bunch of voucher codes every time you create something.
The only thing is it is a bit expensive, but if your paying for custom that's not unexpected
